New Clinic Policies

If prior to your appointment you begin to feel unwell, please call and we will gladly reschedule you.

Please allow adequate time to arrive for your scheduled appointment, if you are late we will need to reschedule you for another available appointment time.

When you arrive at the clinic please wait in your vehicle and call the front desk to notify them that you have arrived.

You will have to remove your shoes upon entering the clinic. Please have socks on for your treatment.

When you enter the clinic our staff will direct you with additional screening procedures that are required for your safety.

If you have a mask available please wear the mask to your appointment. If you do not have a mask we will provide one for a small fee.

Please do not bring extra people to your appointment who are not receiving treatment (parents with children are exempt).

If you are a high-risk individual or a healthcare professional working with immunocompromised people or the elderly, please identify yourself so we can schedule your appointment accordingly.

If you are a frontline worker, please do not come directly from work in your uniform.

At this time, we will not be providing printed receipts. We will email you a copy of your receipts.

If you are a new patient to the clinic, please find the required paperwork on our website: foundationchiropractic.ca and email or fax your completed forms to the clinic: info@foundationchiropractic.ca or fax 306-955-8809. If you unable to print paperwork or fill it out online, please call our office at 306-955-8808.
